Indications/Uses
Replacement therapy in the treatment of hypothyroidism. Suppress TSH production in the treatment of thyroid carcinoma. Diagnostic agent for differential diagnosis of hypothyroidism.
Dosage/Direction for Use
Hypothyroidism Initially 50-100 mcg daily, may be increased by 25-50 mcg at intervals of about 4 wk until thyroid deficiency is corrected & maintenance dose is established. Adult Maintenance dose: 100-200 mcg daily. Elderly, those w/ CV disorder or in those w/ severe hypothyroidism of long standing Initially 12.5-50 mcg daily increased by increments of 12.5 to 25 mcg at interval of about 4 wk. Congenital hypothyroidism 10-15 mcg/kg daily. Childn >1 mth 5 mcg/kg daily, <1 mth 5-10 mcg/kg daily. Adjusted in steps of 25 mcg every 2-4 wk until mild toxic symptoms appear, at which time the dosage is slightly reduced. Juvenile myxedema Childn >1 yr Initially 2.5-5 mcg/kg daily.
Administration
Should be taken on an empty stomach: Take 30 min-1 hr before breakfast. Take at the same time each day.
Contraindications
Hypersensitivity. Thyrotoxicosis. Untreated hyperthyroidism.
Special Precautions
CV disorders including angina, heart failure, MI, & HTN. Introduce gradually in elderly & those w/ long-standing hypothyroidism. Not to be given in patients w/ adrenal insufficiency w/o adequate corticosteroid cover. DM or diabetes insipidus. Thyroid-hormone replacement w/o concomitant treatment w/ corticosteroids may precipitate acute adrenocortical insufficiency in patients w/ impaired adrenocortical function, including those w/ subclinical or unrecognized adrenocortical disease. Lactation.
Adverse Reactions
Anginal pain, cardiac arrhythmias, palpitation, cramps in skeletal muscle; tachycardia, diarrhea, vomiting, tremors, restlessness, excitability, insomnia, headache, flushing, sweating, excessive wt loss & muscular weakness.
Drug Interactions
Increases effect of anticoagulants. May increase phenytoin levels. Enhanced the metabolism of thyroid hormones w/ anticonvulsants eg, carbamazepine & phenytoin. Enhanced effects of sympathomimetic agents. Increases receptor sensitivity to catecholamines. Cardiac glycosides. Reduces GI absorption w/ cholestyramine. Increased dosage requirement w/ OCs.
